After a thorough assessment process, we provide a multi-disciplinary team of professionals to create a Care Plan, which defines the needed community resources. A caseworker is assigned to support the family and will ensure that the measurable goals and objectives are achieved. We develop partnerships with other non-profit organizations to utilize resources that currently exist within our communities today.
